I know how you feel! Alyssa is finally getting the hang of it, after 5 months. But she still won't poop on the potty. After Nate got home she just decided that she would start peeing on the potty and actually telling us when she has to go. Don't get discouraged, she'll learn when she's ready :)
For what it is worth, I had to literally hold Cody on the potty to show him that he would not die from pooping on it. He held it for a couple of days and it was obvious he needed to go. Once he did it, the fear was gone. May not be what you need to do for Avery, but just know many of us have dealt with this one. It is very common.
We let Landon walk around in the house with no bottoms on. He's much more aware of it when he doesn't have anything "protecting" him. Also, go here: http://www.babycenter.com/0_potty-training-in-three-days-or-less_10310078.bc
We haven't done this fully yet, just because of lack of time, but we've applied some of the principles and they really work. We've been doing this only for about maybe 3 weeks and we only use 2-3 diapers a day. Including nap and bedtime.
